In one case, an Air Canada flight nearly landed on a taxiway jammed with four planes awaiting departure.

Both pilots on the flight were very experienced.

Former Inspector General of the U.S. Department of Transportation and aviation expert Mary Schiavo said the cockpit voice recorders capture conversations among the flight crew that are not recorded by air traffic control. Many pilots have said that the global airport is notoriously hard to navigate.

The pilots told the NTSB they were confused about the location of the runway because a parallel runway had been closed for the night. For comparison, the height of a Boeing 787 tail is 56 feet.

On Oct. 22, 2017, another Air Canada flight was cleared to land but a tower controller then instructed the crew multiple times to circle because he was not certain that a preceding arrival would be clear of the runway. "The FAA deemed this event to be an isolated occurrence and not reflective of any systematic deficiencies at Air Canada".

"Upon landing the crew was informed the tower had attempted unsuccessfully to contact the aircraft, however, the message was not received by the crew".

The near-miss incident was not reported to FAA or airport officials until two days later. Federal authorities have determined most of the close-calls reported since December 2016 at the busy San Francisco International Airport were caused by pilots. The FAA's statement to Newsweek said San Francisco International Airport modified its ground radar system to "identify potential conflicts when aircraft are 3/4-mile from the airport".

Last week, the Federal Aviation Administration re-certified the airport after an in-depth review of its operations.

The captain, who was flying the plane, had more than 20,000 hours of flying time, and the co-pilot had about 10,000 hours.

The pilot and co-pilot reported being exhausted after flying from Toronto, according to the NTSB report.

The Air Canada pilot pulled up, and flew over the first two planes by just 30 metres. No conclusions about how or why the overflight occurred should be drawn from the information in the docket, as the investigation is ongoing.

Delucchi said the plane dropped from his radar screen on approach, an unusual occurrence that he said didn't alarm him because he was tracking the plane with binoculars.

Federal officers subsequently issued new guidelines for nighttime landings and control-tower staffing on the airport. Now, when an adjacent runway is shut down at night, air traffic controllers will no longer let pilots make so-called visual approaches to land.

That crew said they too questioned whether they were properly lined up for Runway 28R, but checked with their instruments to confirm the right path.
